OHSAS was formed in January 2001 through a merger of NHS Occupational Health and Safety Services of Fife and Tayside. The merger gave OHSAS autonomy while allowing it to remain within the structure of the NHS.

OHSAS currently employs 150 staff who are based in a number of locations including Rosyth, Dundee, Kirkcaldy, Perth, Stirling and Edinburgh.
